As a patient, you have the right to obtain copies of your records held by the practice. To do this you must make a

Subject Access Request in writing

We are not able to liaise with a friend or family member acting on your behalf without written, signed consent from yourself which actually names your representative.

 

Rights and responsibilities of patients

 

You can express a preference about which dentist you would like to see.  We will make all reasonable efforts to ensure that the request is met, but this may not always be possible.  Please ask the receptionist when you make an appointment about this.

 

You will receive a written treatment plan

 

All staff and patients have the right to be cared for in a safe environment. This practice operates a zero tolerance policy towards aggressive, violent or threatening behaviour, this could result in a patient being refused treatment now or in the future and possibly being reported to the appropriate authorities    

 

Appointments

We are a practice based on prevention if regular check-ups are not maintained you will be removed from our list.

Please note it is your responsibility as a patient to ensure you attend your booked appointments. Our practice policy is that if you fail to attend, or cancel with less than 24 hours notice, on more than one occasion you may be removed from our list.

 

Disabled access

 We have dentist and hygenist services on the ground floor with a ramp to the entrance.

 

Interpreting Service

It is possible for us to access an interpreting/translation service hosted by Bradford and Airedale Teaching PCT.  Please let us know in advance if you require this service. Languages spoken by our staff are English, Urdu, Punjabi, German, Greek.

Should you have a dental emergency outside of the practice opening hours, please call the emergency dental service on 111 who will be able to advise you.

 

Complaints procedure

We hope that most problems can be resolved easily and quickly. If you have any comments or concerns about any of our practitioners or staff,please letus know as soon as possible. This way, problems can often be sorted out at the time they arise with the person concerned.

 

If your problem cannot be resolved in this way and you wish to make a complaint you should write to Dawn Lawrence or your practitioner at the practice address.  Alternatively, you may ask for an appointment with Dawn Lawrence practice manager or your practitioner in order to discuss your concerns. We will acknowledge your complaint within three working days and offer to discuss your concerns with you so that we can agree the manner in which your complaint will be handled and the time scales involved.   A copy of our complaints policy is available from reception.

 

Patient confidentiality

We take patient confidentiality extremely seriously and all personal information is treated in the strictest confidence. We have a confidentiality policy (available on request) and members of staff only have access to patient information when necessary. Patient information is stored on a secure computer system/on manual records which are stored securely.

 

Financial

We accept the following methods of payment at the practice: cash and all major credit and debit cards.

We provide all of the general treatments you’d expect under the NHS, but go further by helping you work towards actually reducing the need for major treatments in the future with ongoing support and guidance in everything from smoking cessation and the effects of alcohol misuse to making sensible dietary choices for you and your family.
